MFS Behavioral Health Benefits and Insurance Information
Metropolitan Family Services is currently a provider for Medicaid, Medicaid MCO Plans, Medicare, MMAI plans and several commercial insurance plans.
Individuals and families who are uninsured or underinsured can access the agency Subsidized Fee Schedule for services based on household size and income. No one is denied services due to inability to pay.

Metropolitan Family Services is currently a provider for the following health plans participating in the Medicare-Medicaid Alignment Initiative (MMAI):

- Humana- MMAI IlliniCare Health – MMAI
- Meridian Health Plan – MMAI
- Molina Healthcare – MMA

Metropolitan Family Services is not accepting new clients for any HMO plans at this time.
Metropolitan Family Services does take Commercial Insurance/PPO plans as an out of network provider after confirmation from the insurance plan.
